REAL SUBMISSION GRAPPLING LIABILITY WAIVER AND RELEASE AGREEMENT
I, {name}, hereby acknowledge and agree to the following:
1. I am participating voluntarily in the Real Submission Grappling program (the “Program”) offered by Real Submission Grappling, LLC (the “Company”) during which I will be engaged in activities including, but not limited to, Brazilian jiujitsu and other forms of martial arts training and conditioning.
2. I understand and acknowledge that participating in the Program involves risks and dangers that may result in harm to me. Such risks and dangers include, but are not limited to, strains, sprains, fractures, cuts, bruises, broken bones, or other injuries, including severe, permanent, or catastrophic injuries or death.
3. I recognize that the Company is not liable for any injury or damage that I may sustain as a result of participating in the Program. As such, I hereby release and hold harmless the Company and its owners, officers, directors, employees, agents, and representatives from any and all claims, demands, suits, or causes of action arising from or related to my participation in the Program, including but not limited to, any injuries, damages, or losses sustained by me.
4. I understand and acknowledge that I have the responsibility to inform Company staff of any pre-existing medical condition and to exercise reasonable caution and care to avoid injuries during the Program.
5. I confirm that I am over the age of 18 years, have read and fully understand this agreement and execute it freely and voluntarily without any inducement by Company.
This waiver shall be binding upon me, my heirs, next of kin, executors, administrators, successors, and assigns.
